Sliding Scale:

Shalem Healing provides a sliding-fee discount for uninsured and underinsured patients. The sliding-scale discount is calculated on your household’s annual income and the number of individuals living in your household. The Department of Health and Human Services revises the Federal Poverty Guidelines each year.

the federal restrictions on providers and facilities regarding balance billing in certain circumstances,
any applicable state law protections against balance billing, and
information on contacting appropriate state and federal agencies if an individual believes a provider or facility has violated the restrictions against balance billing.
